The annual awards application is available online at the link below.  The deadline for submission is June 1, 2025.

Instructions for Applying for Awards:

1. One owner/one dog per form submission. Multiple PWCCA member/owners should submit separate applications if multiple owners desire a certificate. AKC Certifications are not required.

2. Certificates will be printed using the information given on the form. Committee is not responsible for typos on the submission. Dog’s name should be listed EXACTLY as it is to be printed on the certificate to include all titles earned through 12/31/2024.

3. Applicants must be/have been PWCCA member when title(s) were achieved.

4. Deadline for form submission is NO LATER THAN June 1, 2025
